Print the paper version form at your home, fill out it, and bring it to you to your office visit.

It takes about 20-30 minutes to complete the form, please make sure you have sufficient time to finish it.

The initial visit to Dr. Rogers will involve a detailed history in an effort to define the factors causing your allergy or triggering your asthma. A physical examination follows.

Armed with this information, Dr. Rogers is able to determine what allergy tests (skin & sometimes blood tests) are needed to diagnose and confirm the severity of your allergy. Breathing, or pulmonary function tests, are used to evaluate your asthma.

A treatment program can now be designed and tailored to your individual needs. The program may include environmental control, medication or allergy injections, as well as specific patient and family education relating to you.

Telephone Policy Please confine routine calls to office hours. We also request that you have your pharmacy call us directly during office hours for prescription refills. After hours, on weekends and holidays, your call will be taken by our answering service to be forwarded to Dr. Rogers. On rare occasions, another qualified physician will be taking his calls. If your call is not returned within 30 minutes after reaching the answering service, please let us know. IF YOU ARE HAVING EXTREME ALLERGIC REACTION OR ASTHMA EMERGENCY, PLEASE PROCEED DIRECTLY TO A HOSPITAL EMERGENCY ROOM OR CALL 911. THE EMERGENCY ROOM WILL NOTIFY US. Injection Policy Although allergy serum therapy may be quite helpful for controlling your allergies, there are inherent dangers. In spite of all precautions, significant reactions still rarely occur. Most significant reactions occur on an immediate basis; therefore, you are asked to remain in the doctor's office for at least 20 minutes following an injection.
